The Mouth-Brain Connection Could Gum Disease Bacteria Be Feeding Alzheimer’s?
For decades, Alzheimer’s disease has been understood almost entirely as a problem that starts and stays in the brain — the slow, destructive buildup of amyloid-beta plaques and tau tangles that gradually erodes memory and cognitive function. But a body of research building over the past several years has pointed to a surprising possible contributor that starts somewhere else entirely: the mouth.
At the center of that research is Porphyromonas gingivalis, the bacterium best known as a primary driver of chronic periodontitis, or severe gum disease.
How an Oral Infection Could Reach the Brain
Periodontitis damages the soft tissue and bone that support the teeth, and untreated gum disease often leaves inflamed tissue that bleeds easily — creating a direct opening into the bloodstream. Once P. gingivalis gets into circulation, it produces toxic enzymes called gingipains that break down proteins and can help the bacteria evade the immune system.
In a widely cited 2019 study published in Science Advances, researchers led by Stephen Dominy and colleagues examined brain tissue from deceased Alzheimer’s patients and reported finding P. gingivalis and gingipains in a large majority of the samples. In follow-up lab experiments, mice orally infected with P. gingivalis went on to develop hallmark features of Alzheimer’s pathology, including increased amyloid-beta production and loss of neurons.
A Different Way of Thinking About Amyloid
That research fed into what’s sometimes called the “antimicrobial protection hypothesis” of Alzheimer’s — the idea that amyloid-beta isn’t simply toxic waste that accumulates with age, but may actually function as part of the brain’s immune response, produced to trap and neutralize invading pathogens like P. gingivalis. Under that model, repeated low-grade bacterial exposure over years or decades could drive the brain’s immune system to keep producing amyloid, eventually causing collateral damage to healthy neurons.
It’s an intriguing idea, and one that’s shifted how some researchers think about Alzheimer’s prevention — but it’s still a hypothesis, not settled science. Correlation between P. gingivalis and Alzheimer’s pathology doesn’t prove the bacterium causes the disease, and researchers caution that Alzheimer’s almost certainly involves multiple contributing factors, including genetics, cardiovascular health, and lifestyle.
Why the Leading Drug Attempt Didn’t Pan Out
The strongest real-world test of this theory came from a drug called atuzaginstat (COR388), developed by the biotech company Cortexyme specifically to block gingipain enzymes. It’s worth being upfront about how that story actually turned out: in October 2021, Cortexyme reported that its large Phase 2/3 trial of about 650 patients missed its primary goals — the drug didn’t significantly slow cognitive or functional decline compared to placebo across the full study group. A subgroup of patients with higher levels of P. gingivalis in their saliva did show a slower rate of decline on the higher dose, which the company pointed to as an encouraging signal. But a few months later, in January 2022, the FDA placed a full clinical hold on the drug over concerns about liver toxicity, and by that August, Cortexyme discontinued the program entirely and shifted its focus to other diseases.
That outcome doesn’t disprove the underlying gum-brain connection, but it’s a reminder of how far this research still has to go before it translates into an actual treatment — and it’s a good example of why early biological findings don’t always hold up once they’re tested in people.
What You Can Actually Do
While the drug development side of this story has stalled, the practical takeaway hasn’t really changed: taking care of your gums is a reasonable, low-risk thing to do, and it may carry benefits beyond your teeth. Chronic periodontitis is extremely common, affecting roughly half of adults over 30, and it often progresses quietly without much pain until it’s fairly advanced.
A few habits dentists consistently recommend:
- Brush twice a day and floss daily to disrupt the bacterial plaque before it can build up below the gumline.
- Get professional cleanings roughly every six months, since a dentist or hygienist can remove hardened tartar that brushing alone can’t touch.
- Don’t ignore bleeding gums, persistent redness, or chronic bad breath — these are early warning signs of periodontitis, and it’s treatable when caught early.
- Be thoughtful about harsh alcohol-based mouthwashes, which can disrupt the balance of bacteria in the mouth; mechanical cleaning (brushing and flossing) remains the most reliable tool.
The link between oral bacteria and Alzheimer’s is a genuinely active and interesting area of research, and it’s helped push scientists to think about the disease’s origins more broadly. But the field has also just seen a reminder that a compelling biological story doesn’t guarantee a successful drug. For now, the most well-supported move you can make is the one dentists have been recommending all along: keep your gums healthy. If it turns out to help protect your brain too, that’s a bonus — not something to bank on just yet.
